Required Documents for Registration

The following documents are required for registration at Nooksack Valley School District

• Completed Application
• Proof of Age and Legal Name
  • Acceptable documents include a birth certificate, ID card, or passport
• Immunization Records
  • You can access your child’s Washington Statement Immunizations at MyIRMobile - MyIR Mobile, or from your local health care provider, clinic, or local pharmacy.
  • Individual Vaccine Requirements- Washington State Department of Health
• Transcript and withdrawal grades for incoming High School Students
• Proof of Residency within Nooksack Valley School District may be required before the application can be processed and/or finalized
  • Acceptable documents include a power bill, title, or rental contract
